Job summary

PharMerica, an affiliate of BrightSpring Health Services, is seeking a full-time Pharmacist to join our closed-door, non-retail team in Worthington, OH. You will evaluate orders, provide clinical recommendations, and maintain patient profiles within a long-term care setting.

Qualifications

• Bachelor’s Degree in Pharmacy; Pharm D preferred.
• Current and valid pharmacist license in state of practice.
• One year experience in long term care desired.

Responsibilities

• Evaluate pharmacist medication orders for appropriateness, dosage, interactions, and administration.
• Provide clinical recommendations to physicians and healthcare staff on therapy changes.
• Maintain up-to-date resident profiles including medical history and current meds.
• Ensure lab tests for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and healthcare professionals for clinical support.
• Document communications and interventions in medical records.
