Qualifications

• Doctor of Pharmacy (PharmD) degree required or equivalent.
• Active pharmacist license in good standing (state-specific).
• Experience in Medication Therapy Management and pharmacist-led clinical programs.

Responsibilities

• Participate in pharmacist-led clinical programs for chronic disease, quality improvement, and medication optimization.
• Outreach to eligible patients, enroll them, educate, and follow up to maintain engagement.
• Conduct MTM, CMR, and telephonic patient education on medication use and adherence.
• Provide actionable medication recommendations and collaborate with providers to support practice adoption.
• Ensure documentation and compliance with state/federal regulations and HIPAA.
