Qualifications

• Specialized residency or fellowship in infectious diseases or at least 1 year in a dedicated infectious diseases role.
• Pharmacist licensed or eligible in Minnesota and Wisconsin.

HealthPartners is looking to hire a Clinical Pharmacist Infectious Diseases Specialist to join our team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary

The Antimicrobial Stewardship Program (ASP) at HealthPartners and Park Nicollet is designed to enhance patient care through a comprehensive health-systems approach, focusing on improved utilization of diagnostic testing and effective treatment for infectious diseases. The program operates with a central core group comprising four Infectious Diseases (ID)/ASP pharmacists and two ID physicians. The two regional infectious diseases pharmacists lead ASP initiatives in the east and west hospitals and clinics, supported by an ID physician for the west region. A system-wide COpAT (complex outpatient antimicrobial therapy) ID pharmacist provides support across all hospital discharges and infectious disease clinics within the network. The program is jointly overseen by an ID/ASP pharmacist program manager and a system ID/ASP physician leader. An external consulting arm of the ASP offers services to outside entities. The regional ID pharmacists participate in cross-coverage of the COpAT program, system pharmacy manager duties, and ASP consulting.

The two regional ID clinical pharmacist specialists oversee the east and west regions of the health system, including hospitals, clinics, urgent cares and other service areas. The east region pharmacist serves as the ASP lead for Regions, Lakeview, Westfields, Hudson, and Amery hospitals. The west region pharmacist serves as the ASP lead for Methodist, Hutchinson, and Olivia hospitals. These roles work collaboratively with ASP core group, ID physicians, infection prevention, microbiology, medicine, nursing, and information technology to improve the quality and equity of care delivered across our system of 8 hospitals and numerous clinics and urgent cares. They work to implement coordinated quality improvement initiatives in their respective regions and generate data to drive improvements in patient care and sustainable resource utilization. Additional responsibilities may include clinical patient case consultation, education of other staff or patients, promotion of and dedication to evidence-based medicine, leadership of system antimicrobial stewardship initiatives, and standardization of practices across the continuum of care. The ID regional ID pharmacists perform all duties in a manner that promotes the mission and values of the organization and of the ASP.

Work Schedule

Monday through Friday, standard business hours. No holidays

Required Qualifications

Education, Experience or Equivalent Combination:

• Doctor of Pharmacy (PharmD) degree from an ACPE-accredited university OR graduate from a foreign college of pharmacy that has at least a five-year curriculum and has passed the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and become certified by the Foreign Pharmacy Graduate Equivalency Commission (FPGEC)
• Specialized residency or fellowship training in infectious diseases or at least 1 year experience in dedicated infectious diseases role

Licensure/ Registration/ Certification:

• Pharmacist registered or eligible for registration in the states of Minnesota and Wisconsin

Knowledge, Skills, and Abilities:

• Leadership, written/verbal skills, and customer skills are essential.
• Demonstrates knowledge of antimicrobial drug therapy monitoring and infectious diseases treatment principles necessary to assure safety of patients
• Knowledge of antimicrobial agents; this includes pharmacokinetics, pharmacodynamics, notable drug interactions, pharmacology, and appropriate monitoring.
• Computer software: Proficiency in the operation of department related hardware and software.

Preferred Qualifications

Education, Experience or Equivalent Combination:

• 3 or more years' experience in a health-system antimicrobial stewardship program

Licensure/ Registration/ Certification:

• 3 or more years' experience in a health-system antimicrobial stewardship program

Knowledge, Skills, and Abilities:

• Experience with clinical management of primary care based and/or hospital based infectious diseases
• Strong Microsoft Office 360 skills including PowerPoint, Word, and Excel
• Strong presentation and communication skills
