City of Hope in Irvine, CA is seeking a Clinical Pharmacist I on a per diem basis to serve as the medication management expert. The role includes supervising technicians, educating pharmacy staff, and collaborating with providers and nurses to ensure safe, high-quality, cost-effective care for patients.

Qualifications

• Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending.
• Current CA Pharmacist License.
• BLS certification.

Responsabilités

• Collaborates with providers, nurses, and other clinical staff to ensure safe, high quality, and cost-effective medication management for all patients.
• Provide oversight of safe and timely medication use system.
• Oversee compounding in alignment with USP, Board of Pharmacy, and other regulations.
• Ensure medications are received, stored, labelled, transported, and wasted appropriately.
• Ensure controlled substance processes are adhered to.
• Collaborate with the health care team to optimize medication therapy.
• Participates in care coordination and Investigational Drug management for research patients, following regulations.
• Support reporting of Pharmacy Interventions, Adverse Drug Reactions, and Medication Errors.
• Participate in pharmacy staff development, including orientation, students, residents and training.
