Job summary

Allegheny Health Network Home Infusion seeks a pharmacist to coordinate pharmacy services, ensure safe and appropriate care, and support clinicians in delivering medication therapy in accordance with state and federal laws.

The role includes admixture and dispensing, documentation, patient education, and participation in quality improvement, with opportunities to mentor staff and contribute to program growth.

Qualifications

• Pharmacist licensure required or eligible in Pennsylvania with timely licensure within 120 days.
• PharmD or equivalent degree completed from an accredited program.
• Residency in clinical pharmacy or hospital/home infusion experience preferred.

Responsibilities

• Coordinate admixture and dispensing of medications and parenteral solutions.
• Maintain accurate documentation in the clinical record.
• Coordinate medication delivery and supplies to patients.
• Provide drug information to healthcare providers and care teams.
• Participate in Performance Improvement and quality initiatives.
• Lead training and guidance for new staff and develop quality indicators.
