Job summary

Allegheny Health Network is seeking a skilled Pharmacist to coordinate home infusion services, ensuring safe and effective medication management for patients. You will admixture and dispense medications, maintain documentation, and collaborate with care teams to optimize therapy in the home infusion setting.

The role requires a PharmD or equivalent, PA licensure, and a commitment to ongoing education, quality improvement, and patient teaching.

Qualifications

• Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD) required.
• Licensed or eligible for licensure as a Registered Pharmacist in PA at hire or within 120 days.
• Post-graduate residency in clinical pharmacy is preferred.
• Supervisory experience and knowledge of admixture procedures are a plus.

Responsibilities

• Admixture and dispensing of medication and parenteral solutions to patients.
• Dispense pharmaceutical supplies to patients in alternate care.
• Maintain quality control requirements and accurate documentation.
• Coordinate medication and supply deliveries to patients.
• Provide drug information to other healthcare providers involved in patient care.
• Participate in ongoing interdisciplinary communication and performance improvement.
• Develop staff competencies and training, monitor quality improvement data.
• Direct patient/caregiver teaching and physician contact as needed.
