Job summary

Allegheny Health Network Home Infusion is seeking a licensed Pharmacist to coordinate pharmacy services, provide clinical support, and ensure safe admixture and dispensing for patients in the Meadville, PA area.

The role emphasizes collaboration with healthcare providers, documentation accuracy, and participation in quality improvement. A PharmD or Bachelor of Pharmacy is required, with PA licensure or eligibility and residency preferred.

Qualifications

• Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) required.
• Licensed or eligible for PA licensure at hire or must obtain licensure within 120 days.
• Post-Graduate residency in clinical pharmacy preferred.
• Experience with admixture procedures and hospital or home care settings preferred.
• Supervisory experience is a plus.

Responsibilities

• Admixture and dispensing of medication and parenteral solutions to patients.
• Coordinate medication and supply deliveries to patients.
• Maintain quality control and accurate documentation in the clinical record.
• Provide information regarding drugs to other healthcare providers involved in patient care.
• Participate in ongoing interdisciplinary communication and performance improvement activities.
• Direct patient/caregiver education and physician communications as needed.
• Attend seminars/CEU opportunities for professional growth.
• Join on-call schedule as assigned.
