Job summary

Option Care Health in Augusta, GA is seeking an entry-level Clinical Pharmacist to provide safe and effective pharmacy services in a home care setting. You will coordinate patient services with Nursing, monitor progress toward goals, and educate patients, caregivers, and staff about medication use.

The role requires graduation from an accredited School of Pharmacy with an active license. Prior experience in sterile IV admixture or MTM is preferred; travel is limited and adherence to regulatory

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license from the State Board of Pharmacy.
• Experience or training in home care, MTM, and IV/medication management is valued.

Responsibilities

• Assess patient suitability for home care and collect pertinent information.
• Design and monitor patient-specific pharmacy care plans for home care patients.
• Educate patients, caregivers, and staff on medication use and safety.
• Ensure continuity of pharmaceutical care across home and other settings.
• Prepare, dispense, label, and verify medications per policy and regulatory standards.
• Maintain equipment and respond to drug information requests from healthcare providers.
