Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients. You will coordinate with Nursing, educate patients and caregivers, and ensure quality pharmaceutical care across settings.

The role emphasizes understanding medication therapy, regulatory compliance, and effective communication with health care teams. A licensed pharmacist and graduation from an accredited school are required.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Preferred: ASHP residency or Advanced Pharmacy Practice Experience.

Responsibilities

• Assess patient suitability for home care and collect relevant information.
• Design and monitor patient-specific pharmacy care plans for home care patients.
• Modify care plans based on monitoring data and pharmacokinetics where appropriate.
• Provide medication-use education to staff, patients, and caregivers.
• Ensure continuity of pharmaceutical care across home and other settings.
