Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients. The role coordinates with Nursing, educates patients and staff, and ensures continuity of care across settings.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy
• Experience in sterile IV admixture procedures or non-sterile compounding, MTM, or completion of ASHP residency/advanced program

Responsabilités

• Assesses patient suitability for home care per Option Care policies.
• Designs and monitors patient-specific pharmacy care plans for home care.
• Provides medication-use education to patients, caregivers, and staff.
• Ensures continuity of pharmaceutical care across home and other settings.
• Maintains inventory and adheres to DEA regulations and safety standards.
• Responds to drug information requests from health care providers and patients.
• Manages equipment used in medication administration and reports discrepancies.
• Complies with accreditation, regulatory, and safety requirements.
• Maintains confidentiality of patient and proprietary information.
