Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and effective pharmacy services in the home care setting. You will coordinate with Nursing, monitor patient progress, and educate patients, caregivers and staff on medication use.

You will ensure continuity of care, prepare and dispense medications, monitor safety, and comply with regulations while advancing professional competencies through CEU opportunities.

Qualifications

• Experience providing customer service to internal and external customers.
• Basic level skills in Microsoft Word, Excel and PowerPoint.

Responsibilities

• Determines patient suitability for home care and collects necessary information.
• Designs and monitors patient-specific pharmacy care plans for home care patients.
• Provides medication-use education to staff, patients and caregivers.
• Ensures continuity of pharmaceutical care between home and other settings.
• Maintains equipment and participates in medication safety and reporting.
