Job summary

Option Care Health, Inc. is seeking an entry-level Clinical Pharmacist to provide safe, compliant pharmacy services for home care patients. You will work with nursing to monitor progress and educate patients and caregivers on medication use.

Role focuses on designing care plans, monitoring therapy, and ensuring safety and regulatory compliance, in alignment with organizational policies and standards.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience in sterile IV admixture or related MTM/advanced practice
• Willingness to comply with regulatory and safety standards.

Responsibilities

• Assess patient suitability for home care and coordinate with Nursing.
• Design and monitor patient-specific pharmacy care plans for home care.
• Educate patients, caregivers and staff on medication use and safety.
• Ensure continuity of pharmaceutical care across home and care settings.
• Maintain rigorous documentation and compliance with laws and USP standards.
