Job summary
Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients in accordance with policies and regulatory standards. The role collaborates with Nursing to monitor patient progress and educate patients, caregivers, staff and professionals on medication use.
The successful candidate will hold an active Pharmacist license, have customer-service experience, and be prepared to participate in CEU activities and comply with DEA
Qualifications
- Customer service experience with internal/external customers.
- Proficient in Microsoft Word, Excel, and PowerPoint.
- Current, active Pharmacy license from State Board.
- Experience with sterile IV admixture, non-sterile compounding, MTM or residency prerequisites.
Responsibilities
- Determines the suitability of individual patients for home care.
- Designs, recommends, monitors and evaluates patient-specific pharmacy care plans.
- Modifies pharmacy care plans based on monitoring data and lab values.
- Provides medication-use education to staff, patients and caregivers.
- Ensures continuity of pharmaceutical care between home and other settings.
