Job summary

Advocate Health is seeking a part-time Pharmacist at Aurora Lakeland Medical Center - Pharmacy. The position requires 24 hours per week with a rotating schedule including weekends and holidays, and entails ensuring safe dispensing and patient education.

The role includes reviewing prescriptions, counseling patients and caregivers, collaborating with the healthcare team, and supporting formulary management. Experience requirements are flexible, with licensure in state practice and ACPE-accredited

Qualifications

• Pharmacist degree and licensure required; ACPE-accredited school preferred.
• Immunization/injectable certification may be required per practice area.
• Credentialing and privileging based on practice area.

Responsibilities

• Oversees pharmaceutical care and designs pharmacotherapeutic regimens.
• Provides counseling to patients and caregivers on therapy, effects, and adherence.
• Reviews orders, ensures safety, and participates in emergencies and QA.
• Mentors staff and supports medication safety initiatives.
