Aurora Health Care in Wisconsin seeks a pharmacist to oversee pharmaceutical care, design medication therapy regimens, and educate patients and caregivers.

You will work with the healthcare team to ensure safety, compliance, and quality improvement, and may precept students while mentoring teammates.

Requires ACPE-accredited degree (PharmD or B.Pharm with NABP FPGEC), license or reciprocity, and strong computer and communication skills.

Qualificações

• Required: Bachelor's or PharmD from ACPE-accredited school, or NABP FPGEC certificate.
• Licensed in state(s) of practice or eligible for reciprocity; immunization/injectable certification may be required.
• No experience required.
• Strong computer skills, excellent communication and organization, and training abilities.
• Life support training may be required depending on department.

Responsabilidades

• Oversees pharmaceutical care and designs medication therapy regimens based on literature.
• Counsels patients and caregivers on therapy, adverse effects, compliance, and administration.
• Evaluates and implements patient-specific pharmacotherapeutic regimens per policies.
• Reviews medication orders and prescriptions for accuracy.
• Participates in medical emergencies when applicable.
• Supports medication safety and quality improvement activities and reports events.
• Participates in formulary management and acquisition of non-formulary and investigational meds.
• Ensures compliance with regulations and proper storage and handling of medications.
• Serves as preceptor and mentors pharmacy staff and students.
• Directs pharmacy teammates to ensure safe workflows and regulatory compliance.
