Job summary

Qualifications

• Pharmacy degree from an ACPE-accredited school or NABP FPGEC certificate.
• Licensed in the state of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification may be required.
• Credentialing and privileging required based on area of practice.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Counsels patients and caregivers on therapy, adverse effects, and adherence.
• Reviews and interprets medication orders and prescriptions.
• Participates in medical emergency management when applicable.
• Supports medication safety and quality improvement activities.
• Assists formulary adherence and medication-use process management.
• Prepares, dispenses, and stores medications per regulations and policies.
• Serves as preceptor and mentors pharmacy staff and students.
• Complies with all organizational policies and local, state, and federal regulations.
