Mercy Hospital is seeking a Clinical Pharmacist to join our hospital team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective medication therapy and serve as a drug information resource for the care team and patients.

You'll contribute to clinical initiatives, support education efforts, and help deliver high-quality pharmaceutical care aligned with Mercy's mission and standards.

Formación

• BS in Pharmacy or PharmD from an accredited school (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience (preferred).
• PGY-1 Pharmacy Residency (preferred).

Responsabilidades

• Review patient records to ensure appropriate medication therapy.
• Collaborate with prescribers, nurses, and patients on dosing and therapy.
• Develop medication plans to minimize risk and optimize outcomes.
• Provide guidance on drug administration and monitor treatment outcomes.
• Precept and mentor students and residents as needed.
• Participate in Medication Therapy Services and quality initiatives.
• Maintain compliance with regulatory standards and confidentiality.
