Qualifikationen

• BS Pharmacy or PharmD from an accredited school of pharmacy (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience (preferred).
• PGY-1 Pharmacy Residency (preferred).

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to address clinical needs in treatment plans.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and therapy.
