Job summary

HCA Healthcare's Good Samaritan Hospital in San Jose is seeking a Clinical Pharmacist to ensure safe, effective medication use and to support evidence-based practices. You will collaborate with the pharmacy team, nurses and physicians to resolve drug therapy issues.

Requirements include a B.S. or PharmD from an ACPE-accredited school, a California pharmacist license, and at least one year of recent hospital acute care experience; PGY residency and ED acute care experience are preferred.

Qualifications

• B.S. or PharmD from an ACPE-accredited School of Pharmacy
• A current and valid license as a registered pharmacist in the state of California
• One-year recent hospital acute care experience required.
• PGY Residency preferred
• One-year recent ED hospital acute care experience preferred

Responsibilities

• You will have daily interaction with your pharmacy team, nurses, physicians and other facility staff in the hospital and intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions that may arise.
• You will provide medication information and consultation on pharmacokinetics, dose adjustments, orders laboratory tests, medication monitoring according to established policies and procedures
• You will have the opportunity to participate in the training of new pharmacists, pharmacy students, and technicians

Job Summary and Qualifications

As a Clinical Pharmacist, your work plays an important role in the care of patients and healthcare providers by assuring the safe and the effective use of medications as needed. The Clinical Pharmacist monitors and evaluates prescribed therapies to promote optimal drug use and evidence-based practices to reduce risks of drug misadventures.
