Penn Medicine seeks a Pharmacist to collaborate with physicians and other professionals to optimize drug therapy, improve patient outcomes, and manage costs in a hospital setting.

The role involves patient and provider education, drug information support, and participation in safety and formulary activities. Requires Pharm.D. or B.S. Pharmacy and PA licensure; hospital experience preferred.

Competenze

• Pharmacist degree required: Pharm.D. or B.S. Pharmacy.
• Current licensure or eligibility to reciprocate in Pennsylvania required.
• ASHP-Accredited Pharmacy Practice Residency or equivalent preferred.
• Healthcare provider in good standing with Medicare/Medicaid programs.
• Equivalent of three years' hospital pharmacist experience.

Mansioni

• Evaluate drug therapy in collaboration with physicians and allied health professionals to improve patient outcomes and control costs.
• Identify drug therapy strategies to improve outcomes and reduce costs.
• Counsel patients and educate healthcare professionals on safe medication use.
• Develop criteria and recommend medications for hospital formulary.
• Interpret and evaluate medication orders for safe dosing, routes, and schedules.
• Prepare and dispense medications per guidelines and regulations.
• Advise on medication contents, actions, and potential adverse reactions.
• Respond to drug information requests; participate in safety and quality initiatives.
