Penn Medicine is seeking a Pharmacist to collaborate with physicians and allied professionals to evaluate drug therapy and provide medication-related care that improves patient outcomes and reduces costs.

You will monitor responses in complex cases, educate patients and staff, and support medication safety initiatives while contributing to formulary recommendations and safety programs. This role offers a sign-on bonus and hospital setting experience.

Formación

• Pharm.D. or B.S. Pharmacy with equivalent relevant experience required.
• Current licensure or eligible to reciprocate in the State of Pennsylvania required.
• ASHP-Accredited Pharmacy Practice Residency or equivalent experience preferred.
• Equivalent of three 3 years' experience as a hospital pharmacist.

Responsabilidades

• Evaluate drug therapy in collaboration with physicians to improve outcomes and manage costs.
• Identify therapy strategies that optimize patient outcomes and reduce costs.
• Monitor responses to drug therapy in medically complex elderly patients.
• Provide information to patients directly or in teaching materials.
• Conduct drug utilization reviews to assure quality of use.
• Educate healthcare professionals and patients on safe medication use.
