Job summary

Hi-Desert Medical Center, part of the Desert Care Network, is seeking a Clinical Pharmacist Coordinator to lead pharmacy services and contribute to safe, cost-effective medication therapy. You will collaborate with physicians, nurses, and staff to optimize patient care.

Requirements include a CA pharmacist license, PharmD or BS in Pharmacy, BCPS, and hospital residency or equivalent experience. Tenet network affiliation and commitment to professional development are highlighted.

Qualifications

• Possess a valid CA pharmacist’s license (maintain current at all times)
• PharmD or BS in Pharmacy
• Board Certification in Pharmacotherapy (BCPS)
• Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
• Good written and verbal communication skills
• Demonstrate good command of basic mathematical calculations

Responsibilities

• Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
• Attendance on medical rounds (for decentralized shifts) with documentation of pharmacist interventions based on availability.
• Develop and implement facility-specific protocols, guidelines, and reference documents related to medication therapy management to minimize medication errors.
• Conduct Medication Use Evaluations (MUEs) and assist in making changes to pharmacotherapy based on MUE findings.
• Provide support to staff pharmacist and provide all general responsibilities of a staff pharmacist as needed.
• Demonstrate leadership and professionalism with respect to oversight of pharmacy technicians, students, and interns.
• Ensure pharmaceutical care programs and medication therapy management are integrated throughout the institution.
• Maintain compliance with regulatory standards (TJC,FDA,CDPH,CMS,CABoP,ISMP).
• Contribute to department research for pharmacy students and residents.
• Participate in education and training of healthcare professionals in relation to medication use.
