Job summary

Coordinated Care Network is seeking a Clinical Pharmacist to provide clinical oversight, patient counseling, and operational support within our pharmacy and partner clinics. You will ensure safe, effective medication therapy through clinical review, prescriber collaboration, and quality assurance.

You will serve as a liaison between clinics and pharmacy leadership, support workflow efficiency, and conduct site visits to engage with staff and patients.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree.
• At least one year of retail pharmacy experience.
• Currently licensed (unrestricted) as a registered pharmacist in the applicable state.
• Preferred: Specialty pharmacy experience.
• Preferred: HIV-focused counseling and adherence support.
• Preferred: Experience mentoring staff.
• Strong problem-solving and decision-making across clinical and business domains.
• Ability to interpret written clinical and operational documentation.
• Excellent time management and organizational skills.
• Ability to oversee and train employees.
• Strong understanding of pharmacy regulations, fulfillment workflows, and shipping operations.

Responsibilities

• Provide timely, accurate, and comprehensive drug information to nurses, physicians, and other healthcare professionals.
• Counsel patients on medication therapy, adherence, side effects, and disease management; maintain accurate patient profiles.
• Review and interpret prescriber orders for appropriateness and safety.
• Actively participate in patient care activities and clinical decision-making.
• Serve as the primary point of contact for day-to-day clinic issues related to assigned clinics.
• Act as liaison between clinics and the Clinical Pharmacy Supervisor.
• Perform routine site visits to assigned clinics and engage with staff and patients.
• Consult with prescribers for corrections, clarifications, and therapy optimization.
• Monitor daily activities of staff to ensure timely drug therapy and proper caseload management.
• Quality-check all prescriptions on hold for future processing.
• Document and report patient adherence and disease-management activities.
