Job summary

MaineHealth is seeking a Clinical Pharmacist to join the ambulatory pharmacy program within the Hemostasis and Thrombosis initiative. The role focuses on direct patient care, collaborative drug therapy management (CDTM), and coordination with cardiology and primary care teams in an ambulatory setting.

You will work to optimize therapy for thrombotic disorders, educate healthcare professionals, and support transitions of care for clinic patients.

Qualifications

• Education: PharmD or BS in Pharmacy from an accredited school.
• License: Current state pharmacy license.
• Experience: Ambulatory care experience and anticoagulation therapy preferred.

Responsibilities

• Collaborate with healthcare professionals to provide safe, cost-effective patient care.
• Directly communicate with patients and families about medication therapy.
• Optimize anticoagulant, antiplatelet, and hemostatic therapy.
• Educate other professionals and precept trainees on anticoagulation.
• Develop evidence-based clinical pathways with the team.
• Analyze process and outcome measures for therapy effectiveness.
