Location: Buffalo, New York (Remote with travel across New York State)

Duration: 6 Months

Are you a highly skilled and motivated Clinical Pharmacist seeking to make a significant impact on patient care and healthcare outcomes? We are looking for an experienced professional to join our team in a role that emphasizes excellence in medication therapy management, utilization review, and driving quality improvements in healthcare programs.

Responsibilities:

1. Lead initiatives in Utilization Management (UM) and Medication Therapy Management (MTM).
2. Conduct Drug Utilization Reviews (DUR) and provide expert clinical pharmacy consultations.
3. Collaborate with Medicare, Medicaid, Commercial, and Marketplace plans to optimize patient care.
4. Perform Telephonic Comprehensive Medication Reviews and support CMS Opioid DUR initiatives.
5. Drive quality improvements, including enhancing STAR measures and reducing hospital admissions.
6. Ensure strict compliance with CMS standards, confidentiality, and PHI regulations.
7. Deliver expert drug information services and support patient management initiatives.
8. Utilize advanced proficiency in Microsoft Word and Excel to streamline processes.
9. Demonstrate strong problem-solving, interpersonal, and professional collaboration skills.

Qualifications:

1. Comprehensive clinical knowledge and expertise in pharmacy practices.
2. Proficiency in Microsoft Office Suite (Word, Excel) and familiarity with CMS minimum standards.
