Yale New Haven Health seeks a qualified pharmacist to join our Connecticut team, focusing on medication management, safety, and education within a hospital setting.
Qualified candidates hold a PharmD or B.S. in Pharmacy, CT license or eligibility, with ASHP-accredited PGY1 preferred and a minimum of two years of relevant experience. The role emphasizes CHAMP practices and collaboration with the care team.

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
ExperienceCompletion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is preferred, but not required. Minimum of two years of relevant experience specific to practice area is required.
LicensureCurrent Connecticut Pharmacist License, or eligible for pharmacist licensure in the state of Connecticut.
Special SkillsThis individual should demonstrate in depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
