Sound Physicians in Fort Worth, TX is seeking a Clinical Performance Nurse (CPN) to influence care delivery and drive improvements that elevate patient outcomes. This leadership-focused role does not require direct patient care but requires strong clinical knowledge in an acute care hospital setting.

You’ll partner with clinicians to advance quality and discharge planning. The position offers a 40-hour work week, Monday through Friday, with no nights or weekends, and a robust benefits plan.

Qualifications

• Minimum 2 years of acute care nursing experience.
• Active RN licensure in good standing; BSN preferred.
• Proficient with EMR and Microsoft Office.
• Experience with quality improvement initiatives to enhance patient satisfaction.
• Experience in case management or related areas is helpful but not mandatory.

Responsibilities

• Demonstrate strong clinical knowledge in an acute care setting.
• Assess patients' clinical status and readiness for discharge.
• Ensure effective communication across the care team as patients progress.
• Focus on improving the overall quality of patient care.
• Participate in daily workflows and advocate for patient goals in the care plan.
