Job summary

Sound Physicians is hiring a Clinical Performance Associate in Springfield, OR to join a collaborative team focused on improving patient outcomes in a hospital/post-acute setting.

Responsibilities include partnering with nurses and hospitalists, reporting to leadership, and working to prevent readmissions while ensuring a positive patient experience. No direct patient care is required.

Qualifications

• A valid certification or degree as a licensed practical nurse (LPN) or licensed social worker (LSW).
• An active and unrestricted license in the relevant state for this position.
• Three-plus years of experience in hospital or post-acute care settings.
• Proficiency in technology, including electronic medical systems and Microsoft Office.
• Authorized to work in the United States.

Responsibilities

• Partner with Sound nurses, site leaders, and hospitalist team to improve patient care quality and efficiency.
• Report to nursing leadership.
• Improve patient outcomes by promoting health, preventing readmission, and addressing barriers to healthcare access before discharge.
• Work to improve the patient experience and optimize resource utilization.
• No direct patient care.
• Interact directly with patients at the bedside through satisfaction and discharge discussions.
