Qualifications

• Bachelor’s degree in nursing required.
• Current RN licensure in the state.
• Five years nursing experience, with inpatient acute care.
• Leadership/Case Management/Quality experience preferred.

Responsibilities

• Develop workflows to meet program goals and performance targets.
• Provide timely status of implementation and action plans.
• Educate providers on clinical performance objectives.
• Coordinate with hospital departments to ensure workflow delivery.
• Support readmission prevention and transitions of care.
• Share performance data with physicians to drive improvement.
