Job summary

Solaris Health Holdings seeks an experienced Pathway Program Navigation Manager to lead enterprise-wide clinical pathways for urologic conditions, coordinating education, playbook implementation, and adherence initiatives across multiple affiliates and states.

The ideal candidate will oversee nurse navigators, drive quality improvement with data metrics, and ensure standardized, evidence-based care while collaborating with physicians and operational leaders to optimize patient outcomes.

Qualifications

RN licensure required.
BLS certification required.
Graduate of an accredited nursing program required; BSN preferred.

Responsibilities

Lead the development, implementation, and optimization of enterprise-wide clinical pathway and navigation programs across affiliates.
Develop and deliver education and training programs for clinical navigation staff on PCA, BCA, BPH, OAB, and other pathways.
Provide leadership and oversight to nurse navigators and clinical teams to ensure pathway adherence and best practices.
Collaborate with physicians, nursing leaders, pharmacy teams, and stakeholders to integrate pathways into daily practice.
Monitor pathway adherence metrics and outcomes to evaluate program effectiveness and identify improvements.
