Paycom is seeking a Patient Navigator RN to guide patients and families from diagnosis through their care path, helping them understand options and stay well-supported.

The role requires an accredited nursing degree, active RN license, and BLS certification, with at least 2 years of clinical experience. You will collaborate with care teams, maintain accurate records, and ensure HIPAA compliance in a fast-paced environment in South Portland, Maine.

Qualificações

• Graduate of an accredited nursing program.
• Current RN license with state certifications.
• BLS certification.
• At least 2 years of clinical experience.
• BSN preferred.

Responsabilidades

• Guide patients and families through their healthcare experience and educate them on options.
• Follow patients from diagnosis through completion of care path to ensure all options are explored.
• Review prior patient history and keep the medical record updated.
• Coordinate with surgical scheduling on preparations and clearances.
• Raise workflow issues to the manager or director and stay compliant with policies.
• Maintain HIPAA compliance and complete annual training.
• Take on related duties as assigned.

Patient Navigator RNAbout the RoleAs a Patient Navigator RN, you will be the steady, familiar presence for patients and families from the day of diagnosis through the end of their care path. You will help them understand their diagnosis, make sense of their treatment options, and feel supported at every step.
