Job summary

Capital Health is seeking an experienced nursing leader to oversee clinical staff, care management, and operational workflows across its clinics and care settings in New Jersey. The role emphasizes staffing, governance, and high-quality patient care.

The position requires a Bachelor's in Nursing, an active NJ RN license, AHA BLS, and at least five years of long-term care experience with supervisory responsibilities. Master’s degree and multi-state licensure are advantageous.

Qualifications

• Bachelor's degree in Nursing required. Master's degree preferred.
• Five years' clinical long-term care experience, including one year with frail elderly population, and three years of supervisory experience.
• AHA BLS.
• Drivers License.
• Registered Nurse – NJ or Registered Nurse – NLC multi-state.

Responsibilities

• Provides leadership, supervision, and operational oversight of licensed clinical staff and care management personnel across clinic and care settings.
• Directs daily clinical operations and workflow to ensure efficient clinic flow and appropriate staffing levels.
• Oversees care management and clinical support services for assessments, care planning, and care coordination.
• Collaborates with PCPs, Operations Director, Medical Director, and IDT to ensure coordinated, participant-centered care.
• Maintains participation in IDT meetings and communicates participant status changes.
• Ensures staff practice within licensure and policy standards while promoting accountability and teamwork.
• Oversees orders, medication management, documentation, and communication of care plans.
• Develops staffing plans, scheduling, and on-call coverage based on census and acuity.
• Leads clinical team during urgent or high-acuity situations and supports continuity of services.
