Virtua Health in New Jersey seeks a qualified Registered Nurse (RN) to serve as a single point of contact for referring physicians, patients and caregivers, guiding them to clinical and supportive care services.

Responsibilities include coordinating appointments with specialty physicians, educating patients about diagnosis and treatment options, and supporting care continuity across inpatient and outpatient settings. BSN preferred; NJ RN license required.

Qualifications

• Experience in specialty required.
• Problem solving, decision making & critical thinking.
• Excellent leadership, organizational, written & verbal communication & interpersonal skills.
• Must be able to work in a self‑directed environment, with an ability to work with and lead teams.
• Excellent presentation skills.
• Ability to implement professional and community based education programs.
• Database or registry software experience preferred.
• Microsoft Office competency required.

Responsabilités

• Serves as an essential link between patients and all other care providers. Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Adheres to established productivity benchmarks.
• Facilitates appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc. Follows patients throughout the care continuum, including inpatient admissions, and collaborates with inpatient care management resources.
• Actively participates in multidisciplinary planning conferences. Works to collect data, track outcomes, and support strategic planning processes. Delivers reports as requested to Virtua committees and other groups which demonstrate outcomes and performance improvement activities.
• Develops, plans and presents patient education programs and tools.
• Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, etc.)

Summary

Annual Salary

Annual Salary: $79,719 - $123,934
The actual salary/rate will vary based on applicant's experience as well as internal equity and alignment with market data.
