Job summary

Gateway CSB PEO LLC is seeking a licensed clinical professional to assess, plan, and monitor treatment for clients with complex mental health and co-occurring needs. The role includes counseling, discharge planning, and coordinating care with an interdisciplinary team to ensure recovery and community integration.

The position requires a Master’s degree, full licensure or eligibility, and at least two years of relevant experience.

Qualifications

• Master’s degree from an accredited program and full licensure or eligible (LMSW, LAPC, LAMFT, LPC, LCSW, or LMFT)
• Two years of experience in the relevant area
• Knowledge of DSM-IV criteria and diagnosis

Responsibilities

• Conduct intake, screening, assessment, and referral of clients
• Develop person-centered treatment plans and goals
• Provide individual, family, or group counseling
• Coordinate discharge planning and follow-up
• Maintain confidential records and complete required documentation
• Ensure services meet guidelines and are authorized
• Collaborate with interdisciplinary team to support service users
• Meet required direct time and chart audit targets
