We are seeking an LPN/RN with nursing experience in long-term or memory care settings, and a commitment to compassionate, patient-centered care. This full-time position includes a meaningful role in planning, delivering, and evaluating resident care with adherence to

Qualifications

• Current LPN or RN license preferred but not required.
• Experience in assisted living, senior living, long-term care, memory care, or dementia care settings preferred.
• Proficiency with Microsoft Office, including Word, Excel, and email systems.
• Strong written and verbal communication skills.
• Ability to communicate effectively with residents, families, physicians, and team members.
• Strong organizational skills with attention to detail and the ability to prioritize multiple responsibilities.
• Ability to work independently and effectively manage time.

Responsibilities

• Provide nursing care following the individualized resident care plan.
• Medication administration and pain management; timely, accurate administration.
• Assess and document signs and symptoms, change of condition, behaviors and findings.
• Provide nursing services as directed and ensure infection control procedures are followed.
• Assist with staff coverage, supervise and coach clinical staff as needed.
• Maintain professional behavior and patient confidentiality.
