Job summary

Capital Hospice & Palliative Care is seeking an experienced Clinical Manager to oversee patient care coordination, clinical services, and hospice personnel. This role collaborates with the Clinical Director, Medical Director, nurses, and the interdisciplinary team to ensure safe, high-quality hospice care.

The position involves guiding clinicians, reviewing patient progress, supporting QAPI activities, staffing, and training while ensuring adherence to hospice philosophy and patient-centered

Qualifications

• Active RN license in Illinois.
• Hospice or home healthcare experience required.
• Minimum 2 years of management or supervisory experience preferred.
• BSN from an NLN-accredited nursing program preferred.

Responsibilities

• Review hospice referrals and patient information to determine care needs.
• Coordinate patient assignments and hospice personnel.

• Lead clinical record reviews, conferences, and clinician performance evaluations.

About the Position

Capital Hospice & Palliative Care LLC is seeking an experienced Clinical Manager to oversee patient care coordination, clinical services, and hospice personnel. The Clinical Manager will work closely with the Clinical Director, Medical Director, nurses, and interdisciplinary team to ensure patients receive safe, appropriate, and high-quality hospice care.

Key Responsibilities

• Review hospice referrals and available patient information to determine care needs.
• Coordinate patient assignments and appropriate hospice personnel.
• Collaborate with the Medical Director regarding patient eligibility and clinical concerns.
• Review and evaluate patient care through home visits, chart reviews, conferences, and clinician performance.
• Provide clinical guidance and support to nurses and other hospice staff.
• Review patient diagnoses, prognosis, medications, procedures, and clinical progress.
• Assist clinicians with care planning, priorities, and therapeutic goals.
• Participate in interdisciplinary team and case conference meetings.
• Conduct regular clinical record reviews and communicate findings to leadership.
• Assist with interviewing, hiring, orientation, and training of clinical employees.
• Support continuing education and staff development programs.
• Participate in QAPI and quality improvement activities.
• Promote hospice philosophy, quality patient care, and excellent customer service.
• Assist with organizational policies, procedures, and clinical initiatives.
• Perform other duties as assigned by the Clinical Director.

Qualifications

• Current Registered Nurse (RN) license in Illinois.
• Previous hospice or home healthcare experience required.
• Minimum 2 years of management or supervisory experience preferred.
• BSN from an NLN-accredited nursing program preferred.
• Strong clinical assessment and nursing judgment skills.
• Understanding of hospice philosophy and end-of-life care.
• Knowledge of pain and symptom management.
• Excellent verbal and written communication skills.
• Strong leadership, organizational, and interpersonal skills.
• Current CPR certification.
• Valid driver's license, reliable vehicle, and required auto insurance.
