Job summary

Pinnacle Home Care in Winter Haven, FL is hiring a Clinical Manager, RN to oversee clinical operations in the local office, Monday–Friday with potential future hybrid options.

You will triage referrals, oversee care plans, coach field staff, ensure OASIS accuracy, and collaborate with DCS/ADCS to maintain high-quality patient care.

The role requires an active RN license and leadership experience in home health; we offer CEUs, growth, and a supportive team.

Qualifications

• RN license in good standing (state-specific).
• Minimum two years of home health or related field experience.
• Strong leadership and management abilities.
• Excellent written and verbal communication, organizational, and time management skills.
• Knowledge of healthcare regulations and home health standards of care.
• Proficient with electronic medical records (EMR) and healthcare technologies.

Responsibilities

• Triage referrals and align care plans with required disciplines.
• Monitor episodes and coordinate case conferences with clinicians.
• Provide field coaching, onboarding, and performance support.
• Ensure accurate OASIS documentation and care plans; promote Telehealth use.
• Lead regular 1:1 meetings and enforce standards.
• Perform quarterly chart reviews and assist QA tasks.
• Support talent onboarding and candidate screening.
• Deliver in-service training and track completion.
• Model mission and maintain a compliant, patient-focused culture.
• Assist branch leadership with additional patient care and operations tasks.
• Role is not a permanent in-branch position; not for staffing shortages.

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for a Clinical Manager to join our award-winning team.

Key Responsibilities

• Referral & plan-of-care execution: Triage new referrals, confirm required disciplines, align care plans and goals, and update alerts/journal notes.
• Episode monitoring & case conferencing: Track cases through audits and case conferences; be available during operating hours for clinician questions.
• Field coaching: Conduct ride-alongs and field coaching as clinically indicated, including for onboarding, performance remediation, or quality improvement initiatives.
• Documentation & OASIS accuracy: Remove barriers to timely notes, enforce SOPs, and ensure OASIS and care plans reflect current status; promote appropriate Telehealth and Patient Portal use.
• Performance management: Hold regular 1:1s with assigned staff and the Clinical Assistant; reinforce standards, issue corrective actions when needed, and support ongoing competency.
• Quality inputs: Conduct quarterly record reviews; share findings and recommendations to DCS/ADCS; assist in branch QI tasks as directed.
• Talent & onboarding support: Help screen/interview candidates, recommend hires, and execute onboarding checklists and early-tenure coaching.
• Training delivery: Facilitate in-service sessions and education assigned by DCS; track completion and retention of key practices.
• Culture & professionalism: Model mission/values; maintain a respectful, compliant environment with patients, colleagues, vendors, and leaders.
• Other branch support: Perform additional tasks assigned by the DCS/ADCS/Regional Leadership to meet patient care and operational needs.
• This role is not intended to function as a permanent in-branch position, nor to serve as general coverage for staffing shortages absent defined clinical or quality needs.
