Job summary

Elara Caring in Mentor, OH is seeking a Clinical Team Manager to lead patient care and direct care teams in home health. You will coordinate care, staff assignments, referrals, and ensure regulatory compliance.

Responsibilities include reviewing Plans of Care, physician orders, and guiding clinical staff hiring and coaching. You’ll partner with the Branch Director to monitor performance metrics and support field operations.

Qualifications

• Associate degree in Nursing; Bachelor preferred.
• 2 years of home care experience in Home Health or Hospice.

What is Required?

• Associate degree in a Nursing related field, Bachelor preferred
• 2 years of home care experience in Home Health or Hospice environment
• 1 year of supervisory experience required
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
