Pyramid Consulting, Inc. is seeking a Clinical - LTSS Service Care Manager on a 06+ months contract located in Winston-Salem, NC (Hybrid). The role focuses on evaluating member needs, developing long-term care plans, and coordinating with care teams to ensure access to services.

The position requires a Bachelor's degree in a human services field and 2–4 years of related care management experience; RN or LCSW may be required for certain states, with LCSW-A preferred for NC Tailored Plan.

Qualifications

• Bachelor's degree in a human services field (psychology, social work, nursing, etc.) or equivalent experience.
• 2 - 4 years of related experience in care management or service delivery.
• RN or LCSW license required for specific states.
• LCSW-A preferred for North Carolina Tailored Plan.
• Experience in LTSS and HCBS coordination and care management is preferred.

Responsabilités

• Evaluate member needs and available resources, recommending plans for optimal outcomes.
• Develop and monitor long-term care/service plans, identifying necessary providers and community resources.
• Facilitate communication between members, families, and care provider teams to ensure accessibility to services.
• Provide resource support for various needs, including employment, housing, and independent living.
• Conduct ongoing assessments and document care management activities to ensure compliance with regulations.
• Perform home and site visits to assess member needs and collaborate with healthcare providers.
• Educate members and families on healthcare procedures, service options, and benefits.
• Offer feedback to leadership on opportunities to enhance care quality and service delivery.

Pay Range: $38 - $40/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
