Apploi is seeking a part-time Clinical Liaison in New Jersey to cultivate relationships with physicians, discharge planners, and community leaders. This role involves evaluating the clinical status of referrals and promoting engagement with hospitals and local businesses.

The ideal candidate is a resourceful problem solver, flexible, self-driven, and capable of independent work while supporting a team. Prior long-term care liaison experience is preferred.
