Oakdale Seniors Alliance is seeking a Clinical Liaison to provide clinical support and education to patients and families. The role involves conducting assessments, developing care plans in collaboration with healthcare providers, and serving as a key communication link between patients and their families. Ideal candidates will possess an RN or LPN license and have a passion for patient-centered care.
**Commission Structure to Support High Performance**
Oakdale Seniors Alliance is currently seeking a Clinical Liaison to join our team! The Clinical Liaison will be responsible for providing clinical support and education to patients, families, and healthcare providers.
Responsibilities:
- Conduct patient assessments and provide education on various health conditions and treatment options
- Collaborate with healthcare providers to develop and implement patient care plans
- Communicate with patients and families regarding their healthcare needs and concerns
- Participate in case conferences and team meetings to discuss patient care and progress
- Serve as a liaison between patients, families, and healthcare providers
Requirements:
- Current RN or LPN license, preference given for RN license
- Minimum of 2 years of experience in a skilled nursing setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work Monday-Friday and every other weekend on call rotation
