Job summary

Aura Health in Hillsboro, OR seeks a Home Infusion Clinical Liaison to grow program referrals and support transitions for patients entering home infusion. You will build relationships with discharge planners, facility staff, physicians, and families, while ensuring documentation and clinical education are thorough.

Oregon RN licensure with BLS is required; BSN and EMR proficiency are preferred. This on-site role offers comprehensive benefits and opportunities to serve vulnerable populations

Qualifications

• Must hold an Oregon RN license with BLS certification.
• At least 1 year of clinical experience required.
• Preferred: BSN, sales/marketing experience, healthcare background, medical management experience, EMR proficiency.

Role Overview:

The Home Infusion Clinical Liaison supports the growth of home infusion services by building and maintaining positive referral relationships with discharge planners, facility staff, physicians, and patients.

Key Responsibilities:

Assess suitability for home infusion, facilitate care transitions, ensure proper documentation, provide consultative education to providers and families, and serve as a clinical resource.

Qualifications:

Licensure as an Oregon Registered Nurse, BLS certification, and at least one year of clinical experience. Preferred qualifications include a Bachelor's in Nursing, sales or marketing experience, healthcare industry background, prior medical management success, and EMR proficiency.

Additional Info:
