Job summary

Boston Medical Center is seeking a Home Infusion Clinical Liaison to expand referrals and engage providers across the health system. You will coordinate onboarding, support transitions of care, educate patients and families, and drive access to home infusion therapies.

The role requires RN licensure and 3–5 years in home infusion or related fields, with strong relationship-building skills and experience with multidisciplinary teams in a hybrid work setting.

Qualifications

• Bachelor’s degree in Nursing, Healthcare Administration, Business, Marketing, Life Sciences, or related field with 3–5 years in home infusion or related roles.
• RN license required.
• Experience in patient recruitment, provider relationships, or healthcare referral development preferred.

Responsibilities

• Drives referral development and business growth by expanding relationships with physicians, nursing leaders, discharge planners, and partners to increase utilization.
• Coordinates referral intake and onboarding by reviewing, prioritizing, and converting referrals with care teams to start home therapy.
• Facilitates care transitions by serving as primary contact for providers, patients, and families with status updates.
• Educates patients and caregivers on treatment options, procedures, and safety to support compliance.
• Maintains documentation and reporting using EMR and analytics tools to refine referral pipelines and access barriers.
• Delivers clinical trainings and in-services to providers and regional sources on home infusion offerings and insurance criteria.
