Job summary

Acadiana Management Group is seeking a Clinical Liaison for an on-site role in Muncie, IN, with territory near Indianapolis, to coordinate patient referrals and admissions to post-acute LTAC services.

You will evaluate clinical information, collaborate with physicians and hospital staff, and communicate capabilities to referral sources while supporting outreach and relationship-building with healthcare partners and ensuring accurate documentation.

Qualifications

• Clinical coordination and case management experience to assess needs and plan transitions.
• Background in Home Care or community-based services with collaboration across settings.
• Sales and Business Development skills to engage referral sources.
• Knowledge of long-term acute care patient populations.
• Active clinical licensure preferred (RN, PT, LPN, RT or similar).
• Strong interpersonal, communication, and presentation skills.
• Proficiency with electronic health records and standard office software.
• Bachelor’s degree in healthcare or equivalent clinical experience.

Responsibilities

• Coordinate patient referrals and admissions to post-acute LTAC services.
• Evaluate clinical information to determine appropriate levels of care.
• Collaborate with physicians, hospital staff, and community providers.
• Communicate service capabilities to referral sources.
• Conduct patient assessments and explain treatment options to patients and families.
• Facilitate smooth transitions from acute care or home settings to AMG facilities.
• Support outreach and relationship-building with healthcare partners.
• Participate in care planning discussions and maintain documentation.
