New England Life Care (NELC) is seeking a Clinical Liaison Teach Nurse for Brigham and Women’s Hospital in a full-time, on-site role in Woburn, MA. You will assess patients for home infusion suitability, train patients/families, and coordinate care between hospitals and home health teams.

The role involves strong collaboration with hospital staff, documentation, and ensuring HIPAA compliance. Weekend shifts may carry a differential, and quarterly commissions apply after training.

Formación

• BS in Nursing preferred.
• RN license in good standing preferred.
• CRNI preferred.
• Current BLS Certification preferred.
• Knowledge of HIPAA Privacy and Security Regulations preferred.
• Knowledge of The Joint Commission accreditation process preferred.
• Excellent patient assessment skills.
• Working knowledge of home infusion therapy administration/equipment.

Responsabilidades

• Assesses needs and abilities of the patient and caregiver. Participates in developing a patient specific training plan and plan of care.
• Participates in patient training when appropriate.
• Facilitates the discharge process by relaying orders and documentation for discharge to the appropriate NELC departments.
• Acts as clinical resource between NELC and hospital departments (medical staff, care coordination/case management, social services, pharmacy, nursing).
• Assists in coordinating services with other home health agencies.
• Monitors active and pending NELC patient status while hospitalized and communicates patient status to NELC staff.
• Documents all interactions with patient and healthcare team in patient records/electronic progress notes.
• Communicates patient requirements and status changes to infusion and reimbursement teams and HHA staff.
• Participates in daily patient care meetings and case conferences.
• Maintains confidentiality of patient information per HIPAA regulations.
• Completes and documents therapy-specific reimbursement checklists (e.g., Medicare TPN, Medicare ENT, Medicare Cardiac) as required by payer/NELC guidelines.
