Job summary

Jethro Health System is seeking an experienced home health clinical liaison to join our care team in Lynn, MA. You will educate referral sources, coordinate between Home Health and hospitals, and nurture relationships with physicians and facilities to ensure high-quality patient care.

The role requires 2–5 years in Hospice/Home Health with applicable licensure, strong organizational and communication skills, and a proactive, collaborative approach in a fast-paced environment.

Qualifications

• 2–5 years Hospice/Home Health experience.
• Current, valid licensure in MA or ability to obtain (RN/LPN/OT/PT/RT/Social Worker).
• Recent healthcare experience in acute, rehab, or home health settings.

Responsibilities

• Prepares and presents in-services to referral sources and follows up.
• Educates referral sources on Home Health services components.
• Acts as liaison between Home Health and physicians, hospitals, long-term care, and other referral sources.
• Maintains frequent communication with referrals to align patient needs with expectations.
• Resolves issues or dissatisfaction related to quality of care with referral sources.
• Oversees relationships to ensure consistency and quality.
• Documents contacts, referrals, and contracts promptly.
• Supports development of new programs to meet community needs.
• Develops relationships with referral sources like physicians and facilities.

Responsibilities

What You'll Do

• Prepares and presents presentations of various in-services to referral sources and follows-up with the referral sources.
• Educates referral sources on the components of Home Health services.
• Serves as a liaison between Home Health and physicians, hospitals, long-term care facilities, assisted living facilities, and all other referral sources.
• Communicates frequently with each referral source to ensure that the expectations of the referral source and the needs of the patients are being met.
• Assists in identifying and resolving any issue, dissatisfaction, or problem that the referral source is experiencing with quality of care.
• Oversees and monitors primary relationships for consistency and quality.
• Submits, in a timely manner, written documentation of all contacts, referrals, and contracts.
• Assists in the development of new programs to meet the needs of the community.
• Establishes relationships with referral sources such as physicians, hospitals, long-term care facilities, and assisted living facilities.

Qualifications

About You

• You bring 2 - 5 years of Hospice / Home Health Experience
• Current, valid, and active license to practice as a Registered Nurse, Licensed Practical/Vocational Nurse, Occupational Therapist, Physical Therapist, Respiratory Therapist, or Social Worker required in the state where support is provided
• Recent healthcare experience in the acute care hospital, rehabilitation setting, or home health care agency/company required
• You have a proven success in reaching/exceeding quotas
• You have excellent organizational, communication, and interpersonal skills
• You are a strategic thinker who can help evolve our marketing calendars and processes
• You are a self-starter - strong sense of urgency and initiative
• You have the ability to multitask in a fast-paced environment
• You work well with multiple teams and partners Solid computer skills (Excel, Word, PowerPoint, etc.)
• You are a positive collaborator; balances taking initiative and direction, as needed
• You have the ability to work collaboratively with multiple cross-functional teams: sales, marketing, operations, etc.
