Job summary

Heart Healthcare Management PA LLC is seeking a Clinical Liaison / DBD to build and maintain relationships with hospitals, physicians, discharge planners, social workers, case managers, and other referral sources to increase admissions at our skilled nursing and rehabilitation facilities.

The role involves visiting hospitals and physician offices, generating new referral opportunities, guiding referrals through admissions, and educating partners about our clinical capabilities and programs.

Qualifications

• Previous experience in healthcare marketing, business development, clinical liaison work, admissions, or a related healthcare field preferred.
• Skilled nursing, rehabilitation, post-acute, home health, hospice, or hospital experience strongly preferred.
• Understanding of hospital discharge planning and the post-acute referral process.
• Familiarity with Medicare, Medicaid, managed care, and other healthcare payer considerations is a plus.

Responsibilities

• Develop and maintain strong relationships with hospitals, physicians, discharge planners, social workers, case managers, assisted living communities, home health agencies, and other referral sources
• Conduct regular hospital, physician-office, community, and referral-source visits
• Generate and cultivate new referral opportunities while maintaining existing relationships
• Follow up on referrals and assist with the admissions process
• Review referral information and communicate effectively with the clinical and admissions teams regarding patient needs
• Educate referral partners regarding the facility's clinical capabilities, rehabilitation services, specialties, and programs
• Work collaboratively with nursing, therapy, admissions, and administration to support successful transitions of care
• Monitor referral activity and identify opportunities to increase admissions and census

We are seeking a highly motivated and experienced Clinical Liaison / DBD to join its team. As a Clinical Liaison / DBD, you will play a critical role in developing and maintaining strong relationships with hospitals, physicians, discharge planners, social workers, case managers, and other healthcare partners to generate referrals and increase admissions at our skilled nursing and rehabilitation facilities.
