Job summary

Midland Care Connection in Topeka, KS is seeking a Clinical Liaison to bridge Midland with hospitals, physicians, and families. You will coordinate referrals, assess eligibility, and educate providers and patients on available programs, while developing individualized care plans with our care team.

You’ll also support community outreach, maintain documentation, and ensure high-quality, patient-centered care across the continuum in a nonprofit setting.

Qualifications

• Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
• Current professional licensure or certification applicable to the role, as required.
• Experience working in a healthcare setting with patients, families, and/or healthcare providers.
• Strong communication, relationship-building, and customer service skills.
• Ability to assess patient needs and coordinate services across multiple healthcare providers.
• Strong organizational and time-management skills with the ability to manage multiple priorities.
• Proficiency with electronic health records and Microsoft Office or similar computer systems.
• Ability to maintain accurate, confidential documentation and comply with healthcare regulations.

Responsibilities

• Build relationships with hospitals, physicians, and other healthcare facilities to promote Midland’s services.
• Conduct patient assessments to determine eligibility and educate patients/providers about programs.
• Coordinate referrals and develop individualized care plans with the care team.
• Maintain clear communication throughout the care process and with families.
• Support community outreach efforts and documentation for care coordination.
• Ensure patients receive appropriate, high-quality care across the continuum.
