Job summary

Midland Care Connection is a nonprofit, community-based organization in Topeka, Kansas. We provide hospice, home health, grief counseling, and PACE services to help older adults stay independent at home while receiving compassionate care.

The Hospice Admissions Nurse will perform initial assessments, determine eligibility for hospice services, and explain care plans to patients and families. You will coordinate with physicians and the care team to ensure smooth transitions and appropriate

Qualifications

• Active RN license required.
• Graduate of an accredited nursing program (ADN or BSN).
• Current CPR certification.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct initial patient assessments to determine hospice eligibility.
• Explain hospice philosophy, services and care plan to patients and families.
• Collaborate with physicians and the hospice care team to develop an individualized plan of care.
• Ensure medications and equipment are arranged for patient care.
• Support families during the transition to hospice care with empathy and guidance.
