Job summary

Luxor Healthcare in Kansas City is seeking an energetic Clinical Liaison to support our network of skilled nursing and rehabilitation communities. You will build referral relationships with hospitals, physicians, and case managers to drive admissions and occupancy growth.

Youll coordinate patient transitions, conduct facility presentations, and work across hospital and community partners to ensure seamless care transitions and positive outcomes.

Qualifications

• 2+ years of admissions, business development, clinical liaison, or related healthcare experience.
• Experience in skilled nursing, rehabilitation, post-acute care, or hospital settings preferred.
• Strong communication, organizational, and relationship-building skills.
• Professional customer service and phone skills.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, case managers, and community organizations.
• Review referrals and coordinate admissions processes, including assessments, documentation, and authorizations.
• Conduct facility presentations and tours for referral sources and families.
• Communicate resident needs to facility teams to ensure smooth transitions.
• Promote a positive image of our facilities within the community.
• Maintain knowledge of Medicare, Medicaid, and Managed Care processes.
• Travel between facilities and throughout the Kansas City market to support referral development and admissions growth.
